He did not tolerate Xyrem due to worsening depression, and he subsequently underwent bilateral DBS of the GPI, with surgical results pending at the time of this writing. Thompson et al57 found no effect of clonazepam on essential tremor. Clozapine can substantially improve essential tremor, but its use is limited as it can cause fatal agranulocytosis.58 Botulinum toxin has been used in the treatment of essential tremor of the hand, voice, and head. It has been found to improve tremor severity significantly without improving function. In a randomized clinical trial of 25 patients with essential tremor of the hand, significant improvement in tremor severity rating scales was reported.59 It was concluded that botulinum toxin has modest tremorlytic effect.

- In an open label trial, 12 patients with essential tremor were in a group receiving clozapine, with doses ranging from 18 mg to 36 mg.141 Seven had a marked improvement in tremor, two had mild improvement, and three had no benefit.
Accurate diagnosis of tremor is important because appropriate treatment depends on the accuracy of the clinical diagnosis. This article reviews the classification and management of tremor. Many patients do not respond to them, and those who do may not have a significant improvement in their daily life. The side effect profile of drugs used in essential tremor may limit both monotherapy and polytherapy. Two post-mortem studies of coeliac disease patients with cortical myoclonus have demonstrated selective loss of Purkinje cells, illustrating that isolated cerebellar pathology can generate cortical myoclonus [62,63]. In EPM-1 (Unverricht-Lundborg disease), another disorder with prominent cortical myoclonus and EtOH-response, a post-mortem study showed a similar loss of Purkinje cells with involvement of the dentate nucleus [64,65].
Essential tremor: diagnosis and management
For people with tremor alcohol will temporarily improve symptoms of tremor. In some studies it has been estimated that 50%, and as much alcohol and essential tremor as 80% of people according to the U.S. National Institutes of Health, will see an improvement of symptoms after drinking alcohol.
Sometimes, ancillary testing such as brain imaging or genetic testing may help with the diagnosis. However, one theory suggests that your cerebellum and other parts of your brain are not communicating correctly. There’s no cure for essential tremor, but the symptoms can be managed in several ways, starting with practical measures such as those listed above in “Tips for surviving essential tremor.” If you are taking medications for ET, it is important to be careful about your alcohol use. Some drugs to treat ET have an interaction with alcohol which can be dangerous. Alcohol is known to improve the symptoms of ET because of its impact on some brain chemicals that doctors have identified as causing tremors.
The effect of alcohol on essential tremor
Delirium tremens is the most severe form of alcohol withdrawal, and is also characterized by a coarse tremor and signs of heightened autonomic activity, often with hallucinations or delusions. Cerebellar signs set in following more prolonged exposure to alcohol. The vermis shows degenerative changes, extending through all three cortical layers.
When tremors are noticeable, the condition is classified as essential tremor. For many, the late actress Katherine Hepburn provided an indelible public image of essential tremor. Her quavering voice and trembling hands unmistakably betrayed the disorder. After drinking, you may see improvement in your tremor within about 15 minutes. The fact that ET often gets better after you drink alcohol can be useful if your doctor is trying to find out what type of tremor you have.
Effect of Alcohol on Tremors
Neuroleptics or dopamine antagonists cause classic parkinsonian tremor. Long-term neuroleptic treatment can result in a postural tremor with a frequency of 3 to 5 Hz, but is also present at rest and during goal-directed movements. Lithium intoxication can cause a cerebellar tremor.37 Treatment is withdrawal or dose reduction of the offending agent if possible.

Gamma-hyroxybutyric acid (GHB) is a derivative of GABA with similar effects to EtOH. GHB is found as an endogenous molecule within the brain, although at very low concentrations. Sodium oxybate (Xyrem), the sodium salt of GHB, has been studied as a potential treatment for refractory alcohol-responsive movement disorders. Like EtOH, GHB reaches a peak dose within 35 minutes of administration, and plasma levels show a direct, non-linear dose response.
